What is a Clinical Team Report?

A Clinical Team Report, or CTR, is a legal document signed by an MD, doctoral level psychologist and licensed social worker. Each clinician meets the individual for whom guardianship is being sought, reviews clinical documentation for the individual, and documents that the individual would be best served by having a guardian. Trusted Clinical Supports provides the psychologist and social work signatures and the completed CTR.

Who would sign the CTR as the MD?

Trusted Clinical Supports provides the person petitioning for guardianship with a completed CTR, with signatures from the psychologist and social worker. The person seeking guardianship would then bring the CTR to the individual’s Primary Care Physician, pediatrician or psychiatrist to review and provide the MD signature. Typically, the MD reviews and signs the CTR prepared by Trusted Clinical Supports.

Of note: The CTR needs to be filed in court within 180 days of the individual being seen by the MD, psychologist and social worker. If your loved one has not been seen by his/her/their PCP or psychiatrist for a number of months, you may want to let them know you will be applying for guardianship and determine if an appointment is required.

Why am I receiving two copies of the completed CTR?

The Court requires the CTR be printed on a specific type of paper, and that all signatures be original. The three signatures (MD, psychologist and social worker) do not, however, need to be on the same page. You will be sent a copy on the court required paper, and an additional copy to be reviewed and signed by the MD. It is strongly suggested the original CTR not be left at the MD’s office for signature - the additional copy can be used for this.

Will insurance pay for the CTR? How will I be billed?

Insurance does not pay for the CTR. You will receive an invoice with the completed CTR.
